What should you include?

You may not have any strong feelings or preferences about your own funeral, it can still be very useful to those arranging your funeral to know this.

You may decide not to complete all of the sections; they may not all feel relevant, or you may have more information than you can fit in the boxes, but it will hopefully give you somewhere to start.

It is not always easy to start the conversation about our own funeral and mortality. This form may be a useful conversation starter.
